This is a supervisory and administrative position that oversees a 24/7 Mental Health Crisis Call Center. The Program Manager provides supervision, leadership and training to counselors. Case Manager (Homeless Recovery Services)

Profile:

The Case Manager, as part of the Bell Services Team and in collaboration with the person served, is responsible for providing assessment, service plan development and supportive housing services to residents of the Bell Building. The Case Manager will provide flexible supportive services to residents of the Bell Building that will assist them to maintain their housing and promote their recovery. The Case Manager provides services to an assigned case load. Case Manager will be required to bill MCPNs/Medicaid in electronic record system, and will be under the direct supervision of the Program Manager of Bell Services (QMHP or QMRP credentials) and also for billing purposes.

Profile:

This position involves the provision of outreaching nursing care and physical health assessments of older and younger OBRA persons living in various specialized residential settings, the coordination of nursing services with those of the treatment team members pursuant to an individual plan of service and the routine education of direct care staff. This position also provides implementation of all infection control processes and in-service training to NSO employees.

Profile:

This position provides a broad range of case management services for the Year-Round Youth Services Program. The Case Manager will have a high level of engagement with program participants in order to provide intake assessment, ongoing monitoring, support and advocacy to youth. This position develops and coordinates program services including outreach and support services. Counsels and aids youth requiring assistance, including but not limited to problems such as personal and family adjustments, mental health, substance abuse, violence prevention work readiness, social and life skills. Profile:

This position entails the provision of direct services to the residents of the NSO-OAS Residential units. These services are para-professional in nature and include (but are not limited to) such activities as assisting residents with ADLs, providing transportation, assisting with scheduling and keeping appointments, passing medications, assisting with planned activities, and cooperating and assisting clinical staff in the implementation of treatment plans. The position also requires knowledge and implementation of agency and unit policies and procedures, including such topics as Safety and Infection Control, as well as guidelines regarding timeliness, call-in procedures and appropriate interactions with the residents.

Hospital Services Liaison Placement Specialist

Profile:

This is a direct service position relating to the provision of indicated professional mental health services to public clients residing within designated public or contractual psychiatric hospitals. This position involves the provision of indicated services to public care patients from the point of admission to the hospital to the time of discharge. The responsibilities include the assessment of psychiatrically hospitalized persons to independently determined the need for treatment, the determination of whether inpatient psychiatric treatment can be adequately provided in other settings to minimize financial burden to the client and to utilize available insurance coverage agency, participation in team meetings within psychiatric hospitals, ongoing clinical contact, and discharge planning activities, and other assignments as directed.

Profile:

The PATH Case Manager, as part of the Tumaini Center – Housing Services Team and in collaboration with the person served, is responsible for providing assessment, service plan development, housing placement and assists clients in the application process for SSI/SSDI using the SOAR process. It is estimated that 25% of work time will be used to assist clients using the SOAR process. This position is a community based position which involves intensive outreach services. FUSE Peer Mentor (Homeless Recovery Services)

Profile:

FUSE Peer Mentors will support residents who are moving from homelessness to success and satisfaction with permanent housing and community participation. The FUSE Peer Mentor will support recovery and participation by sharing experiences with and educating about recovery and rehabilitation, coordinating with internal and external resources, and walking with the person on the path of recovery and community participation, independence, and productivity. The FUSE Peer Mentor also acts as a liaison with the public utilizing his/her own experience to educate and inform about mental illness.

Accounting/Finance Intern-Unpaid

Profile:

Performs general basic accounting functions such as assisting staff with preparing financial reports, maintaining ledgers, journal entries and other activities associated with budgets and assets. Input journal entries into general ledger for accounts payable and receivables as directed. Reviews cash receipts against the financial status report.

Monitor (Tumaini)

Profile:

This position provides screening and monitoring of homeless consumers and all incoming packages. Position monitors and assists with the general physical maintenance of the building; including monitoring and providing security throughout the inside and front of the building as well as the parking lot. Staff will also provide support and encouragement to consumers, assist in maintaining a safe and secure environment and assist with addressing the basic needs of consumers.

Requirements:
