Profile: |
This is a ancillary supports coordination position. Duties include planning, orchestrating, and coordinating outreach to school age people with developmental disabilities for the purpose of educating students, parents, teachers, and others about services available for persons with developmental disabilities with a goal of increasing the amount of school age consumers served by NSO Life Choices Unit. Administrative responsibilities involve regular face to face contact with persons with developmental diabilities. Telephone calls and e-mail messages must be answered within one business day. A significant amount of the work is performed in the community including homes, school, and other sites where consumers receive services. |

Profile: |
This is a clinical and administrative position. Clinical duties include those of the Outreach Therapist position and involve the provision of psychotherapeutic services to older persons living in a variety of settings and to adult mentally-ill persons living within nursing homes and homes for the aged. Administrative responsibilities include the supervisor of outreach staff, case consultation, routine daily activity consultation, acting as liaison between Outreach Coordinators and Outreach Therapist, students, volunteers and other pertinent tasks related to program operations as well as other assigned related responsibilities. |

PATH Case Manager (Homeless Services) |
Profile: |
The PATH Case Manager, as part of the Tumaini Center – Housing Services Team and in collaboration with the person served, is responsible for providing assessment, service plan development, housing placement and assists clients in the application process for SSI/SSDI using the SOAR process. It is estimated that 25% of work time will be used to assist clients using the SOAR process. This position is a community based position which involves intensive outreach services. The PATH Housing Specialist/Case Manager provides services to an assigned caseload under the supervision of the Program Director. |

Must possess or be able o pass the CAAC exam within six months of employment. Must have a limited license in social work or psychology and be able to secure a full license within one year of employment. Must take the following courses within the first three months of employment: - CCISC training course (as outlined by clinical supervisor) - Communicable Diseases - HIV/AIDS and substance abuse - Usage of CareNet (for the Bureau & SEMCA) - Usage of the ECO system (internal database to NSO) Also, requires computer literacy, with particular emphasis on Microsoft Office Products and Internet Explorer as well as must develop a plan within the first month of employment to take the NSO sponsored CCISC training series appropriate for the position.
