This position provides case management, follow up and variety of support services to homeless consumers. Support services include, but not limited to: screening for mental heal issues, including dual diagnosis, substance use disorder, substance abuse treatment, educational opportunities, preparation of the consumer to live independently in their own home. The Case Manager will also provide housing search assistance and assistance with gathering and compiling the documents necessary to apply for various subsidized housing programs and crisis intervention services. Assistance with completing applications for housing will also be provided. The Case Manager will have a high level of engagement with the consumers.
